ITEMS REQUIRED WITH APPLICATION -Proof of U.S. Citizenship: Certified Birth Certificate or previous U.S. Passport -Proof of Identity: Driver’s License -Recent 2x2 Color Photograph: (maybe taken at Wal-Mart or B & W Graphics) Fees -Book (cost is $110.00 for adult payable by check or money order) -Card (cost is $30.00 for adult payable by check or money order) -Book and Card (cost is $140.00 for adult payable by check or money order) -Service Fee (cost is $35.00 payable by check, money order or exact cash) FEES ARE LISTED ON DEPARTMENT OF STATE WEBSITE AT travel.state.gov. BY LAW, THE PASSPORT FEES ARE NON-REFUNDABLE. PASSPORT VALIDITY LENGTH If you are 16 years of age or older: Your U.S. passport will be valid for ten (10) years from the date of issue except where limited by the Secretary of State to a shorter period. If you are under 16 years of age: Your U.S. passport will be valid for five (5) years from the date of issue except where limited by the Secretary of State to a shorter period. |
